This one changed with the June 22 update: a purely static photo used to be a guaranteed Mimic tell, but it no longer is — shooting a patient over a static photo now might just knock out a normal patient instead of confirming a threat. It's still an anomaly, and letting it in triggers a Death Ritual, but static alone is a weaker signal than it used to be. The safest play post-update is simply shutting the window rather than shooting on a static photo alone.

Treating slime or a static photo as automatic proof is a common beginner error. Patients dropping slime are NOT necessarily anomalies, and static photos stopped being a guaranteed Mimic tell after the June 22 update — don't shoot based on static alone anymore.
You have spotted it — now what
You are holding a developed Polaroid, and the patient is still outside, which shapes the answer: you have the evidence in hand before it is through the door, which is the best position you can be in — refuse at the window.

Refuse it. Shutting the window costs nothing and ends the problem. It does not hurt your shift score, and it is always available.

Or put it down. A confirmed anomaly can be tased or shot, and doing so is worth +2 sanity. A wrong treatment also kills one with no sanity penalty — it only lowers the end-of-shift reward.

What it costs to guess. Tasing or shooting a normal animal by mistake is −15 sanity, nearly a quarter of the 61 you start with. That gap is the whole reason to confirm before acting rather than acting on a hunch.
An anomaly cannot be knocked out the way a normal patient can, so "it fainted, it must be fine" is not a test. Neither is slime on the floor — patients drip it too.

Once you are already looking through the Polaroid camera, these cost you nothing extra to rule out. Knowing them as a set is what turns the check from a habit into a decision.
The polaroid doesn't match the patient standing at the window — different eye shape, different ear shape, or different ear color. A common example: bunny ears in person, but bear ears in the printed photo.
