The Animal Hospital tier list, ranked by what carries a shift

Ranking here is about survivability and coin throughput on a real shift, not about which class looks best. A class earns S Tier by keeping you alive deep into the late shifts or by paying for itself quickly; it lands in C Tier when its perk is real but narrow. Prices are included because the cheapest genuinely good option matters more to most players than the theoretical best one.

S Tier

  • Surgeon2,500 Coins

    Sanity + speed boost after every heal; L3 doubles the sanity gain. The best class you can buy with coins โ€” and therefore the best class, period.

  • Secret AgentRobux (price varies โ€” see note)

    Post-rework kit (re-verified July 16): a gun with 20 uses, a multi-use scanner and +2 backpack from level 1; 24 rounds with 1 regenerating per round at level 2; and +4 sanity plus $5 per anomaly kill at level 3 โ€” anomalies become income.

Sources disagree on the current Robux prices for Head Nurse and Secret Agent (they appear to have changed at least once since launch), and the July 10 rework may have shifted Security and Secret Agent's power level โ€” early videos suggest Secret Agent was buffed. We list only what's cross-verified and will update prices and rankings after re-testing rather than guess.

A tier list is only worth reading if you can see the reasoning behind each placement. Below is every class in ranked order with its signature perk and the verdict from its own class page โ€” the same judgement the tiers above are built on. Read it as an argument you are free to disagree with: if you play a role the perk suits better than we assume, move the class up.

Two patterns fall out of the table. The classes that rank highest convert something you were doing anyway โ€” checking patients in, curing them, shooting anomalies โ€” into sanity, and sanity is what actually ends runs. The classes that rank lowest have perks that are real but narrow: an extra backpack slot or a speed boost โ€” the B-tier Nurse and Paramedic โ€” helps every shift without ever being the thing that saves it. What this tier list costs in Animal Coins

Ranking a class is one thing; affording it is another. Animal Coins come from three actions โ€” curing a patient pays 2, blocking an anomaly pays 0.5, and each visitor handled pays 0.5 โ€” and there is no shortcut, because the game has no code redemption system at all. Priced in cured patients at 2 coins each, the recommended path costs 60 cures for Secretary, 250 for Psychologist and 1,250 for Surgeon โ€” 3,120 coins and roughly 1,560 cures end to end. Doctor sits at 900 coins (about 450 cures) and Security at 1,250 (about 625) if you want them on the way, while Nurse costs 20 coins โ€” about ten cures, which is a rounding error next to anything else on the ladder.

Only two entries on this list have no coin price at all: Secret Agent in S tier and Head Nurse in A tier are Robux-exclusive. Sources disagree on what they currently cost โ€” see the note above before budgeting for either.
