Added in the July 25 Hazmat update, this is the newest entry on the list. Size alone is the check: no normal patient spawns anywhere near this small. It also tends to stack other tells on top — a static photo and a warped body on CCTV are its most common companions — so a second-channel check usually confirms what your eyes already told you.
Tiny Anomaly is on the cheapest channel there is — you catch it by looking at the patient, with no tool and no walk to the monitors. That is true of only 8 of the 19 anomalies in the game, so treat the free ones as the exception rather than the rule.

You are at the check-in window with the patient in front of you, which shapes the answer: the shutter is already within reach, so refusing is the cheapest move available — it costs no sanity and no ammunition.
Refuse it. Shutting the window costs nothing and ends the problem. It does not hurt your shift score, and it is always available.
Or put it down. A confirmed anomaly can be tased or shot, and doing so is worth +2 sanity. A wrong treatment also kills one with no sanity penalty — it only lowers the end-of-shift reward.
What it costs to guess. Tasing or shooting a normal animal by mistake is −15 sanity, nearly a quarter of the 61 you start with. That gap is the whole reason to confirm before acting rather than acting on a hunch.
An anomaly cannot be knocked out the way a normal patient can, so "it fainted, it must be fine" is not a test. Neither is slime on the floor — patients drip it too.
Once you are already looking through your own eyes, these cost you nothing extra to rule out. Knowing them as a set is what turns the check from a habit into a decision.
