When players say "ending," they usually mean the questline conclusions:
Barney's two endings โ hide him (second coffee machine reward) or turn him in (Duckman gets stabbed, no reward)
Ratthew's farewellโ his final visit leaves you the key to the Room 5 safe and the game's deepest lore reveal
Liz's questline โ 4 UV photos across Shifts 11โ21, a Chocolate Bar after the 3rd, and a Mimic ambush hiding behind the 4th
These resolve their own stories but don't end the game โ shifts continue either way.

Some sites claim an ending triggers after roughly 15โ20 shifts. We found no verifiable evidence for thisโ no recorded footage, no wiki documentation, no developer statement โ and it contradicts how the endless structure demonstrably works. High-shift runs like "Shift 100" are endurance challenges at pinned max difficulty, not a road to a cutscene. How a run actually ends
The game never ends on its own, so the only endings it hands you are failures โ and there are exactly two of them. That's the real answer to "how do you beat Animal Hospital": you don't beat it, you survive it, and these are the two ways to stop surviving.

Three animal deaths
Three patients die during the run. Dr. Harlow spells this rule out on his last active visit, Shift 6.

You start at 61% by default (higher with the right class) and bleed it to jumpscares, eye contact and bad calls.
Only with help โ a teammate can revive you with a $1,000 Teddy Bear, and you can't use one on yourself.
Where the three animal deaths usually come from
A Mimic that reveals itself mid-treatment or in recovery and kills the patient โ that kill counts against you as an animal death.
A Bed Monster you leave alone: if it's ignored or you're stuck, it drags the patient to death within 30 seconds.
A mistake in Room 8's Tentacle sequence. Run out of time or break the order and the patient dies outright โ and the taser, gun and coffee are banned during it, each one an instant kill.
Tasing or shooting a normal animal, which also costs you โ15 sanity. This is the reason the anomaly tells are worth learning properly rather than rejecting on a hunch.
Deaths also charge you twice: a Ghostspawns after a patient dies and drains โ2 sanity per second while it's near you โ so the mistakes that walk you toward three deaths are the same ones that walk you toward the other fail state. Full numbers are in the sanity cheat-sheet.
Barney's two endings, step by step
Of the three story routes, Barney's is the only one where what you decide changes what you get โ which is why it's the one people mean by "ending". Barney appears on Shifts 2โ7, up to three visits per shift, and works through a fixed chain of requests before the decision lands. Note that tasers and guns do nothing to him โ there is no third option where you deal with him yourself.
Coffee โ four cups across the whole storyline.
The red briefcase โ he leaves it in your care.
The photograph โ he asks you to photograph him at the booth. The print is the first look at his true form: a wide, sharp-toothed mouth and hollow eyes.
The scalpel โ he asks you to hand him one.
"Hide me" โ he asks to be hidden behind the front desk, because the police are coming.
Then, on Shift 8 or later, Officer Duckman arrives holding the photo you took, hunting the serial killer in it. Nothing before this point locks anything in โ refusing an individual request doesn't cost you the chain. This is the fork:

Ratthew's farewell โ the ending with no choice in it
Ratthewis the other storyline players call an ending, and unlike Barney's there is nothing to decide. He first turns up around Shifts 2โ4 and returns across roughly five visits, giving up a little more each time: he checks in anonymously, then tells you his name, then warns you to trust no one in the hospital, then admits he uncovered a government secret and is being hunted for it โ and mentions his missing twin sister Rattina, who worked at Animal Corporation before she vanished.
On his last visit he says he has to disappear, and leaves a key and a note at reception. The key opens the safe hidden behind the cat poster in Room 5, at the far back-right of the hallway. Inside: a photo of Rattina, a suitcase stamped TOP SECRET, and a note reading "the experiment was a success. The shadow now sees everything."
That is the deepest lore payoff in the game so far, and it is also a real practical loss: while Ratthew is in the office he warns you in red text whenever an anomaly approaches check-in. His goodbye takes that free second opinion away for the rest of the run. Nothing about the shift structure changes โ he simply stops coming.
Liz's questline โ four photos, one of them a trap
Liz arrives around Shifts 11โ12 and stays part of your run through roughly Shift 21, so her camera is in your hands for the next 8โ10 shifts. She hands it over and asks for photographs of specific patients โ four in total, each capturing a different stage of the anomaly infection. Spend a photo on the wrong patient and she takes the camera back for the rest of the run.

What "endless" actually changes after the story stops
Endless doesn't mean unchanging. A few things are one-off or early-only, and knowing which is which is most of what separates a Shift 20 run from a Shift 8 one:
Your boss leaves.Dr. Harlow's last active visit is Shift 6 and his end-of-shift grades run through Shift 7 โ after that nobody is checking your work, or reminding you of the rules.
The ambulance event becomes the rhythm. It first lands at the end of Shift 4 โ six patients at once, any of them possibly anomalies โ and then repeats every 5 shifts for the rest of the run.
The shop stays on a timer.From Shift 3 the Supplies Shop opens for 3 minutes at the start of each shift with 3 random items. Late shifts don't give you more shopping time, only more to spend it on.
Some threats never scale. The Head Banger only appears on Shifts 2โ5, so a mechanic you learn early simply stops mattering. The Stalkershows up roughly once per run and can't be killed at all โ it never becomes a fight, no matter how deep you go.
What does scale is density. The same threats arrive more often and faster, which is why the community rule of thumb tops out at solo play being comfortable to about Shift 6.
