Where you meet itDisguised as a patient. It can reveal itself at check-in, mid-treatment, or during recovery โ€” often right after muttering "I'm hungry...". Can appear in Rooms 1โ€“8 or at the front desk.
Damageโˆ’15 sanity on the initial attack, then โˆ’6 sanity per 0.5 seconds while it's actively attacking you. If it kills a patient, that counts against you as an animal death.
How to counter itA single gun round, or a taser, puts it down. Bare-handed fighting also works but drains sanity fast โ€” faster in a group. A Bed Monster can also kill it. It's slower than you, so kite it toward the front desk taser if you're caught without a weapon.

How the Animal Hospital Mimic works

The Mimic is the game's most common late-reveal threat โ€” it's the thing several visual anomalies (Staring, Twitching) always turn into during recovery, and the thing the camera-only Disguised Mimic anomaly shows in its true form. Two field-tested tells apply here specifically: a hunched, slouched idle posture is a near-certain sign, and listening for "I'm hungry..." muttered right after treatment gives you a warning before it turns.

What the Mimic costs, in sanity you actually have

The raw figure is โˆ’6 every 0.5s while it is attacking. What matters is what that is a fraction of: with no class bought you start a shift on 61 sanity, and reaching zero puts you down โ€” a teammate can revive you with a $1,000 Teddy Bear, but you cannot use one on yourself.

  • As a drain it runs at 12 sanity a second, so a full 61-sanity bar lasts about 5 seconds of continuous exposure.
  • On a Psychologist every one of those numbers doubles โ€” that class starts at 100% sanity but takes double from everything, so this threat is exactly twice as expensive there.

What you want in your hands for the Mimic

Worth knowing before you spend a shift carrying the wrong thing: a weapon is the answer to only 3 of the 10 threats in this game. The Mimic is answered by more than one:

Gun

One round puts a Mimic down. On a Ghost it only works once the fire extinguisher has forced it visible.

Carrying it is not a single-purpose decision โ€” the same item also handles the Hiders and the Ghost.

Taser

The front-desk taser is the fallback when you get caught without a gun โ€” the Mimic is slower than you, so you can reach it.

Carrying it is not a single-purpose decision โ€” the same item also handles the Hiders and the Ghost.

When you can expect the Mimic

The nine threats split into three planning problems. The Mimic is in the group that can arrive at any point โ€” it has no shift gate and no trigger you control, so the only defence is being equipped before it happens.

The others in that bracket are the Bed Monster, the Stalker and the Hiders. Grouping them this way is more useful than a flat list, because a threat you can wait out, a threat you set off yourself and a threat that can arrive unannounced each call for a different habit โ€” and only the last one justifies carrying gear all shift.

Catch it earlier: the anomalies that turn into the Mimic

You do not have to fight this if you spot it at the window. Each of these is a patient you can still refuse โ€” and each betrays itself in a different way, which is why the channel matters as much as the tell.

Its head tracks you โ€” or a random target in the room โ€” constantly. In person the tracking is limited to roughly 60ยฐ, but on CCTV there's no limit at all, so the head can swivel further than a real neck should allow.

  • Head constantly tracks you or a fixed target
  • In person the tracking is limited to roughly 60ยฐ
  • On CCTV there is no limit โ€” the head swivels past a real neck's range

How to be sure: A head rotating past what is physically possible on camera is unambiguous.

Spasming arms or head, a stretched neck, and a hunched back. Sometimes the twitching is only visible when you check the camera feed rather than looking at the patient directly.

  • Spasming arms or head
  • Stretched neck
  • Hunched back

How to be sure: The twitch can be camera-exclusive โ€” check CCTV before you commit to treating.

Shows its true Mimic form only on the camera feed. In person it looks completely normal all the way up until it reaches check-in.

How to be sure: Use the lobby camera to catch it from the side before it ever reaches your window.

โš ๏ธ Common mistake

Punching a Mimic bare-handed costs a pile of sanity and barely works solo โ€” use the taser or gun, or gang up in a group instead.

Does any class make the Mimic easier?

2 classes start you holding something this threat responds to, which removes the scramble to find one mid-shift:

  • Security

starts every run with an X-Taser โ€” 5 charges at level 1, 6 at level 2, and a charge refunded each round at level 3.

  • Secret Agent

starts with a gun โ€” 20 rounds at level 1, 30 at level 2, plus a round back each round at level 3.

One to avoid for this specific fight: the Psychologist doubles every sanity effect in both directions. It starts you at 100% rather than 61%, but it also makes this threat cost twice what the numbers above say โ€” a trade that only pays if you are confident you will not be touched.

FAQ

How much sanity does the Mimic cost?

โˆ’15 sanity on the initial attack, then โˆ’6 sanity per 0.5 seconds while it's actively attacking you. If it kills a patient, that counts against you as an animal death.

What's the correct way to deal with the Mimic?

A single gun round, or a taser, puts it down. Bare-handed fighting also works but drains sanity fast โ€” faster in a group. A Bed Monster can also kill it. It's slower than you, so kite it toward the front desk taser if you're caught without a weapon.

Is the Shapeshifter the same as the Mimic?

Yes โ€” Mimic is the current official name (the wiki's canonical page title). The creature was originally called Skinwalker until the July 12, 2026 update renamed it. "Shapeshifter" was a widely-used community name that stuck across YouTube, TikTok and older guides. All three names refer to the same creature.

Where does the Mimic appear in Animal Hospital?

Disguised as a patient. It can reveal itself at check-in, mid-treatment, or during recovery โ€” often right after muttering "I'm hungry...". Can appear in Rooms 1โ€“8 or at the front desk.

What item do you need for the Mimic?

Gun or Taser. One round puts a Mimic down. On a Ghost it only works once the fire extinguisher has forced it visible. The same item also answers the Hiders and the Ghost.

What's the most common Mimic mistake?

Punching a Mimic bare-handed costs a pile of sanity and barely works solo โ€” use the taser or gun, or gang up in a group instead.
