🎭 Shapeshifter

Disguised as a patient. It can reveal itself at check-in, mid-treatment, or during recovery — often right after muttering "I'm hungry..." or "I feel funny...". Can appear in Rooms 1–8 or at the front desk.

Damage

−6 sanity per 0.5 seconds while it's actively attacking you. If it kills a patient, that counts against you as an animal death.

Counter

2 gun rounds, or a taser, puts it down. Bare-handed E-spam also works but is slow — faster in a group. It's slower than you, so kite it toward the front desk taser if you're caught without a weapon.

The Shapeshifter is the game's most common late-reveal threat — it's the thing several visual anomalies (Staring, Twitching) always turn into during recovery, and the thing the camera-only Disguised Shapeshifter anomaly shows in its true form. Two field-tested tells apply here specifically: a hunched, slouched idle posture is a near-certain sign, and listening for "I'm hungry..." or "I feel funny..." muttered right after treatment gives you a warning before it turns.

⚠️ Common mistake

Punching a Shapeshifter bare-handed costs a pile of sanity and barely works solo — use the taser or gun, or gang up in a group instead.
