How the Animal Hospital Stalker works

The Stalker is the one threat in the roster you're never meant to fight โ€” the entire mechanic is about denying it eye contact, not damaging it. The origin of the Stalker is one of the game's unresolved lore threads: the note found in Ratthew's Room 5 safe ("the shadow now sees everything") is widely read by the community as describing exactly this creature, though that connection isn't officially confirmed.

What the Stalker costs, in sanity you actually have

The raw figure is โˆ’10 per eye contact. What matters is what that is a fraction of: with no class bought you start a shift on 61 sanity, and reaching zero puts you down โ€” a teammate can revive you with a $1,000 Teddy Bear, but you cannot use one on yourself.

  • At 10 sanity a time, 6 of these empties a default 61-sanity bar โ€” the 7th puts you on the floor.
  • On a Psychologist every one of those numbers doubles โ€” that class starts at 100% sanity but takes double from everything, so this threat is exactly twice as expensive there.

Nothing in your inventory resolves the Stalker โ€” it is one of the threats you handle by behaving differently rather than by carrying the right item. That is the norm rather than the exception here: a weapon answers only 3 of the 10 threats in the game.

When you can expect the Stalker

The nine threats split into three planning problems. The Stalker is in the group that can arrive at any point โ€” it has no shift gate and no trigger you control, so the only defence is being equipped before it happens.
