How the Animal Hospital Tentacle works

Tentacle is the one encounter in the game where your usual toolkit actively works against you — the taser/gun/coffee instincts that save you everywhere else are the exact wrong move here. This is also the event referenced in the beginner mistake below: forgetting that Room 8's first surgery step is always IV drops, and that Tentacle specifically bans the taser, gun and coffee, is one of the more common ways players lose a patient they were otherwise handling correctly.

What the Tentacle costs, in sanity you actually have

This one does not touch your sanity bar at all — kills the patient; costs you no sanity directly. That makes it easy to underrate: you walk away feeling fine while the run quietly moves closer to the three-animal-death limit that ends it. Treat the cost as a life, not a number.

What you want in your hands for the Tentacle

Nothing in your inventory resolves the Tentacle — it is one of the threats you handle by behaving differently rather than by carrying the right item. That is the norm rather than the exception here: a weapon answers only 3 of the 10 threats in the game.

When you can expect the Tentacle

The nine threats split into three planning problems. The Tentacle is in the group you set off yourself — it does not roll at random — a specific event brings it out, which means the counter-play starts before it exists.

The others in that bracket are the Ghost and the Camera Monster. Grouping them this way is more useful than a flat list, because a threat you can wait out, a threat you set off yourself and a threat that can arrive unannounced each call for a different habit — and only the last one justifies carrying gear all shift.
